Do I need to fill out any patient forms before my first appointment?
To make your registration process run smoother, please print and fill out the forms included in our New Patient Packet. If you are being treated for prostate cancer, please also fill out this questionnaire to give you and your physician an accurate understanding of the severity of your symptoms.
What should I expect at my first appointment?
Your first appointments at RCK will involve registration and meeting with your care team and doctor. Please arrive 15 minutes early to complete the registration process. Generally, the first appointment can take 1 – 2 hours, so please plan accordingly as we want to ensure we have plenty of time to get all your questions addressed and provide you guidance on next steps.
Registration:
When scheduling your first appointment, the receptionist will ask you if you want to register for Noona, our patient portal, or if you prefer us to email or mail you your intake forms in advance. This will allow you to complete your forms at your leisure (but prior to coming in for your appointment).
During registration, a patient services representative will take a photo for your medical record and will review forms and information with you, including:
- Demographic information
- Consent forms
- Completed questionnaires. Be sure to fully complete these, including your cell phone number and email address
- Your insurance benefits
- Patient Rights and Responsibilities document
- Copy of your driver’s license
- Copy of your health insurance card(s)
- List of medications you are actively taking
Meeting with your doctor and care team: Your doctor and care team will review your medical history and perform a physical exam. Further testing may be scheduled. Your doctor and care team will discuss next steps with you and your family. Your care team may include:
- Nurses
- Medical Assistants
- Radiation therapists
- Physicists & Dosimetrists
- Financial Counselors
- Schedulers
When will I receive my treatment plan details?
During your consultation, your radiation oncologist will make a recommendation for your radiation treatment plan. Once you have signed consent to proceed with radiation treatment, they will then schedule you accordingly. Each patient’s treatment plan can be different, with some requiring additional imaging and/or additional procedures prior to the treatment planning CT simulation.
Next, you will have your CT simulation, which your team will use to design your treatment plan. It takes the radiation oncologist, physicists, and dosimetrists some time to craft your custom treatment plan. About 1-2 weeks after your CT simulation appointment, the technician will begin scheduling your treatments.
